SEGREGATION CELLS RECORDS


37. A director shall have a log book maintained in the segregation cells area of his correctional centre in which shall be recorded
 (a)
the name of each inmate confined in a segregation cell, the date and time his confinement began, and the date and time his confinement ended,
 (b)
the name of the officer who ordered the inmate confined in a segregation cell,
 (c)
the date and time of a temporary absence of the inmate from his segregation cell, the reason for the absence, the name of the officer authorizing the absence, and the date and time of the inmate's return to segregation cell,
 (d)
the date and time of a visit made by a person to the inmate, the name of the person making the visit and, where that person is an officer or an employee in the branch, the position in the branch he holds,
 (e)
the names of each officer in each shift supervising the segregation cells, and
 (f)
the date, time, and summary of details concerning any unusual occurrence occurring in the segregated cell area.
