(a)

the date and time the inmate contracted the illness or injury that caused death, | |

(b)
|
the date and time this illness or injury was first reported to a medical officer, | |
|
(c)
|
the apparent cause of death, | |
|
(d)
|
the date and time of death, and | |
|
(e)
|
any other comments that the medical officer considers relevant concerning the death. | |
