Take notice that an application will be heard by the Chief Justice of the Court at Queen's Bench of New Brunswick at _______a.m. / p.m. on ________the _______day of _________19 ____, at the Justice Building, _______________ , in the Province of New Brunswick for
(a) an order designating a Judge to empanel a jury to hear the application, a copy of which is attached hereto, and to determine whether the applicant's number of years of imprisonment without eligibility for parole ought to be reduced;

Before the hearing of his motion, the respondent shall provide to the Chief Justice

(b) a complete list of the institutions where the applicant was imprisoned on this charge, together with the dates of entry to and transfer from each institution.
